When you replace one belt replace all 3. Why? Because its a pain to do. PS, maybe your belt problem is related to your other problems. Max

Reasons for belt popping off.  Oily belt, worn belt, bad water pump....etc.  I guess I should start with the belt.  A new belt.  Funny thing is I had a loose belt for months.  Would chirp a bit at start then be OK.  I finally tightened just a bit.  No chirp.  Been good for many months.  Maybe I over tighten and ruin belt.  Anyway a new belt is in the cards.  Pulleys look good and seem to be in alignment.
