Go to Mercedessource and look at their video on saving rust spots. Clean it up and use Miracle Paint, maybe over some patch if necessary. Small stuff can be saved. Take the door panel off anyway and give it a good once over. I think most everyone has some rust in their door bottoms. But, in the west we normally don't have too many holes.
