Since you have 285K miles and the brushes wore unevenly, it may be that one of the slip rings has worn badly or is pitted. If you have it off the car, you might try removing the voltage regulator / brush assembly and look down the hole the brush came out of to see the condition of the slip rings and compare them. Slowly rotate pulley, so the ring scans by where you are looking.
( I think we are talking about the same thing. I think that on a generator or starter, it is a commutator and it is made up of 20 or so little strips of copper. I think in a Bosch alternator, there are two rings that go all the way around the shaft. I think that is called a slip ring. Corrections, anyone?)
