The lake is most likely water from the AC's in dash evaporator , there's a drain tube that clogs with dust , mud dauber wasp nets or simply rots away occasionally (it's rubber) .
The center vents have a flapper that can remain closed for several reasons , as I'm in Las Vegas where it reached 107* F & was still 104* F @ 6:00 PM , I walked into the Dollar Store across the parking lot from where SWMBO & her childhood friend were shoe shopping and bought a $1 , 12" long screwdriver , got back in the car with the paper windshield shade in place and shone my flashlight down the center vents until I could sort of see the flapper (when it's closed it's horizontal) and ever so gently poked at it , it turns out the trailing edge closest to you , raises up 1.5" or so , then you can slide a dowel (I used my tire pressure gauge) to hold the flapper open and when you turn on the AC again , dust will blow onto your clean shirt along with mass quantities of ice cold air =8-) .
I hope this helps , I was *really* afraid of breaking the flapper or other plastic / rubber parts in there but I was _dying_ in the heat
